Occupational Risk Analysis at PT. Blambangan Bahari Shipyard Using FMEA and 5S-Based Risk Control
Main Article Content
Abstract
This research aims to analyze occupational accident risks at PT. Blambangan Bahari Shipyard using the Failure Mode and Effect Analysis (FMEA) method and to formulate risk control recommendations based on ISO 45001:2018 and the 5S concept. The research focused on the fabrication and sub-assembly workshops, which involve intensive production activities and significant occupational hazards. Data were collected through direct observation, interviews, and questionnaires distributed to workers. Risk assessment was conducted using the FMEA parameters of Severity (S), Occurrence (O), and Detection (D), which were used to calculate the Risk Priority Number (RPN) and determine risk priorities. The results identified 40 potential hazards, consisting of 30 low-risk, 2 medium-risk, and 8 high-risk categories. The dominant hazards were associated with the repetitive use of electrical tools, exposure to fiberglass dust, workplace noise, welding activities, and electrical hazards. Based on these findings, risk control recommendations were formulated by applying the ISO 45001:2018 hierarchy of controls and supported by the implementation of the 5S (Seiri, Seiton, Seiso, Seiketsu, Shitsuke) concept through improved work discipline, proper use of personal protective equipment, workplace organization, and housekeeping practices. The proposed recommendations are expected to minimize occupational accident risks and contribute to the development of a safer, cleaner, and more organized working environment.
